Set aside 1-hour with our dietitian to start your journey to better health. Whether you’re managing blood sugar, blood pressure, or weight—take control by balancing your meals, tracking your progress, and getting the support you need to live healthier.

Diabetes Management Program
Are you ready to make a change to gain control of your A1C?* Join our dietitian and pharmacist in this 6-session nutrition therapy and medication management program. Set yourself up for a lifetime of success by learning the facts about diabetes and the role of food, medication, and other lifestyle factors.
Session Include
- 100% coverage for eligible Select Care members (requires pre-certification)
- (6) one-on-one sessions with a registered dietitian and/or registered pharmacist
- Dexcom continuous glucose monitoring
- Personalized meal plan
- Up to $225 Pay-Less gift certificates
- $25 IP&E gas gift certificate upon program completion *On average, participants see a 1%-point drop in A1C along with reduced diastolic blood pressure within 3 months
How to Register

1.) Visit our online scheduling web page.
- 2.) Select a nutrition service.
- 3.) Select your preferred location.
- 4.) Select your preferred date and time. - For services that require pre-certification, please select a date that allows at least (7) business days for processing.
- 5.) Complete the General Details form.
- 6.) Finalize your registration by adding your Signature and selecting: Book Appointment
We will contact you to confirm the status of your pre-certification and confirmation of your booking.
